Recent advances in Desirability Of Outcome Ranking (DOOR) Analyses

risk evaluation. A patient’s outcome is ranked based on pre-specified clinical criteria, where the most desirable rank represents a good outcome with no side effects and the least desirable rank is the worst possible clinical outcome. Herein, we aim to extend the DOOR methodology in two frontiers

first, by considering the temporal nature in the construction of DOOR and generalizing the analysis to repeated measures and survival analysis

second, by offering different covariate adjustment techniques that could assist researchers in understanding clinical or observational data. For each proposed extension, the steps to conduct analysis and draw asymptotic inference are described, along with demonstrations in simulation studies and real-world data applications.

A clinical trial aims to uncover high-quality evidence on both efficacy and safety of an experimental treatment. Traditionally, statisticians evaluate the two aspects marginally, where, for example, efficacy considers the proportion of patients cured and safety considers the proportion of patients not showing adverse events. However, separate analyses may yield misleading results, as the cumulative nature of clinical outcomes and the correlation between efficacy and safety endpoints are neglected.
